Schedule - Shift: 8 hours

Requirements - Current state license as a physical therapist - Required - Proof of current CPR certification - Required - Current driver's license. Must have reliable transportation and insurance - Required - 1-2 years experience as a physical therapist in an acute care or rehabilitation setting - Required - Previous home care experience - Preferred - Bachelor of science in physical therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities - Perform clinical assessments to identify patient needs and appropriateness for therapy. - Document, prepare, review, and complete a plan of care (POC), progress notes, and clinical record documentation regularly. - Develop and implement treatment plans for neuromuscular, neurological, and orthopedic disorders based on evaluations and medical referrals. - Instruct patients and families on transfer techniques and the safe use of rehabilitation equipment. - Provide patient and family education regarding therapeutic procedures tailored to individual needs. - Collaborate with an interdisciplinary team to coordinate patient care.
