Dietitian: Diabetes Care & Nutrition Lead

Family Health Care Centers of Greater Los Angeles, Inc. is seeking a Registered Dietitian to provide comprehensive nutritional care within our General Medicine/Diabetes Clinic.
The role includes patient assessment, education, care coordination with PCPs via NextGen EHR, and developing care plans to improve outcomes. The position reports to the Chief Medical Officer, presents opportunities to lead and train staff, and supports diabetes care programs and nutrition policy updates.
