Senior Social Worker - Geriatric Care & Discharge
Skilled Healthcare Center
Skilled Healthcare Center is seeking a dedicated Social Worker to provide psychosocial support to residents and families, promoting emotional well-being and assisting with adjustment to long-term care placement. You will participate in care planning, discharge planning, and interdisciplinary collaboration, while ensuring compliance with CMS and state policies.
