• Monday to Sunday : 10:00 AM – 6:00 PM
• Total: 8 Hours per day (Can be filled by one or multiple caregivers)

Job Description
We are seeking compassionate, reliable, and detail-oriented caregivers to assist a client transitioning home from a skilled nursing facility. This role is vital in ensuring the client remains safe, comfortable, and supported during their recovery.

The ideal candidate will be comfortable following a structured care plan and maintaining accurate clinical logs.

Key Responsibilities

• Personal Care: Providing essential assistance with showering, toileting, and dressing.
• Mobility Support: Assisting the client in and out of bed and ensuring safety during transfers.
• Clinical Tracking: Recording daily vitals in a Weekly Clinical Log.
• Detailed Care: Following a specific Morning and Evening Daily Care Checklist to ensure all needs (nutritional, hygiene, and comfort) are met.

Requirements

• Proven experience in home care or a similar healthcare setting.
• Strong communication skills and an empathetic approach to senior care.
• Ability to maintain clear and accurate daily documentation.
• Reliable transportation to ensure punctuality for scheduled shifts.
• Georgia CNA.PCA, or HHA certification, preferred.
