Responsibilities:

• Provide skilled nursing care to patients in their homes
• Complete OASIS assessments and required documentation
• Manage patient care plans and collaborate with interdisciplinary teams
• Educate patients and families on disease management and recovery goals
• Deliver high-quality, patient-centered care in the community

Requirements:
✅ Active RN license
✅ Previous Home Health experience required
✅ Strong OASIS documentation experience
✅ Ability to work independently in a field setting
