Patient Care Liaison (In-Market Leadership Role) Sweetwater Care | Skilled Nursing & Post-Acute Services Lead the Market. Build the Team. Drive the Results. Park Place Transitional Care is seeking a Patient Care Liaison who doesn’t lead from a desk you lead from the field. This role is for a proven performer who knows how to drive census, build referral pipelines, and elevate teams . You’ll be responsible for both your own market production and coaching Patient Care Liaisons to perform at a higher level. If you’re currently a top-performing liaison ready for the next step or a regional leader who misses being in the action this is your role.
