First (Days) Overview: Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse. Overview: The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being. Anticipated Status/Shift: Full Time (40 hours/week) Day Shift Position Location: Sentara Hospice Services - Chesapeake, Virginia Beach, Norfolk, Portsmouth Pay Range: $22.39 - $37.32/hour We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Education

- Trade School Graduate for Nursing (Required)
- LPN License in state of practice (VA) (Required)
- BLS (Required within 90 days)
- 2 years' experience (Required)
- Serve as patient advocate in the prevention of illness or disease (Preferred)
- Provide care and treatment for patients and clinical support and teaching for patient and family members (Preferred)
- Customer service, testing, medication administration and the implementation of nursing interventions and care plans (Preferred)
- Support a holistic approach to patient care (Preferred)
