Nurse Practitioner/ Physicians Assistant

Make a lasting impact by helping patients avoid unnecessary hospitalizations while building long-term, meaningful relationships. Physician Services Group (PSG) is seeking a Full-Time Nurse Practitioner (NP) to join our Post-Acute Care team to support our Lake City and Mayo FL locations.

Travel is required between the two facilities.

\n

This is a Monday–Friday role offering consistency, clinical autonomy, and a supportive team-based environment.

Schedule & Setting

\n

• Full-Time | Monday–Friday | 8:00 AM–4:00 PM
\n
• In-person care at a local Post-Acute & Skilled Nursing Facility
\n
• Collaborative care alongside attending physicians and on-site teams
\n

Compensation & Benefits

\n

\n
• Salary: $100,000–$120,000 annually (based on experience)
\n
• Annual performance-based bonus
\n
• $1,500 CME stipend + paid holidays
\n

Qualifications

\n

\n
• Active Nurse Practitioner license in Florida (or ability to obtain)
\n
• Board Certified (ANCC, AANP, or equivalent)
\n
• Strong clinical judgment and documentation skills
\n
• Experience in post-acute, hospital, or outpatient care preferred
\n
• Passion for geriatric medicine, preventative care, and chronic disease management
\n
