We are seeking experienced Registered Nurses (RNs) with strong assessment, care coordination, and member education skills to support a Disease Management program. Candidates with prior Case Management experience, telephonic care management experience, and Medicaid knowledge are highly preferred.

Required Qualifications:

• Active Pennsylvania RN License

• Disease Management experience

• Telephonic Case Management experience

• Strong assessment, care coordination, and member education skills

Preferred Qualifications:

• CCM (Certified xhyhwjd Case Manager)

• Medicaid experience

• Previous Case Management experience

Additional Requirement:

• Candidates must complete the Care Manager Assessment in Glider.
