Responsibilities include: Managing and coordinating hospice patient care Performing comprehensive patient assessments Developing and implementing individualized care plans Educating and supporting patients, families, and caregivers Collaborating with interdisciplinary hospice teams Providing compassionate care that promotes comfort, dignity, and quality of life What Makes This Role Unique? As a Regional Support Hospice RN, You Will: Deliver high-quality, compassionate nursing care for hospice patients Manage and implement individualized plans of care Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) Assess and address patients’ physical, emotional, spiritual, and environmental needs Accurately document all care provided and ensure compliance with hospice protocols Educate families and caregivers on care instructions, medications, and safety Participate in the on-call rotation and provide after-hours care as needed Collaborate with physicians, referral sources, and the care team Assist with referrals, regulatory documentation, and payer compliance Travel to various Gentiva hospice locations as assigned to fill urgent needs Schedule & Travel: This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include: Regional travel with overnight stays based on assignment needs. A company fleet car is provided for eligible employees.
