Nurse Practitioner, Paneled

Paneled Primary Care Nurse Practitioner Humana’s Primary Care Organization is seeking a full‑time Nurse Practitioner to manage a dedicated panel of adult and geriatric patients in a value‑based care model. Responsibilities Manage and provide primary care services to a designated panel of senior patients. Drive panel performance through quality metrics (HEDIS, STARs, risk adjustment) and patient engagement initiatives. Coordinate care across interdisciplinary teams, including specialists, behavioral health, and care management. Report to the Associate Medical Director. Work fully onsite in Homeland Park, SC. Required Qualifications Graduate of an accredited Nurse Practitioner program (MSN or DNP). Current national certification (AANP or ANCC). Active, unrestricted NP license in South Carolina. DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire). Preferred Qualifications 2+ years of Nurse Practitioner experience. Experience managing a patient panel in primary care or value‑based care. Background in Medicare Advantage or geriatric‑focused care. Experience with Athena EMR. Familiarity with HEDIS quality metrics, STAR ratings, and Medicare guidelines. Benefits Competitive compensation package including sign‑on bonus, relocation assistance, and comprehensive benefits. Outpatient clinical setting focused on continuity and preventive care. Standard schedule Monday–Friday 8:00 AM – 5:00 PM. Pay Range $136,200 – $187,400 per year.
