The Licensed Practical Nurse (LPN)

The Licensed Practical Nurse performs a wide variety of duties and responsibilities in a manner that places emphasis on quality of care and customer service. They provide services directly and collaborate with providers as well as outside agencies to ensure a patient centered approach to care.

As part of the essential functions of this role, the Licensed Practical Nurse:

• Collaborates with and supports team members to ensure a patient-focused approach to care
• Provides outstanding customer service while maintaining a compassionate, positive, and respectful attitude toward people who use drugs
• Assesses, plans, implements and evaluates care for patients under their scope of practice. Prepares patients for provider visits by taking vital signs, history and assessing risk factors
• Collects and records health information findings of physical and developmental assessments into patient record with timeliness and accuracy; accesses and retrieves medical records via HEALTHeLINK™
• Responsible for screening all new and returning patients by completing an intake process; comprehensively documents intake encounters and health history into our EMR system
• Facilitates lab and diagnostic tests such as injections, urine and oral toxicology's at the request of the provider

Qualified Candidate will have current LPN license and current BCLS certification with a minimum of one (1) year of experience working in a medical practice setting, preferably in an HIV/AIDS or other infectious disease practice. Prior phlebotomy experience required; candidate must be able to communicate clearly and professionally in writing and verbally; must demonstrate familiarity with computers and MS Office software (Outlook, Word) and electronic charting/medical records systems (Medent preferred); demonstrate personal responsibility and integrity and ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
