OPS ADVANCED PRACTICE REGISTERED NURSE - 64901321

The position is for a lead provider to manage the health care for 30 of our more complex and high-risk patients. This position will act as lead medical provider for a new "Intensive Case Management Program" and will include a separate clinic, collaborating closely with the other free clinics currently at GRACE. We anticipate operating 4 hour in-person clinic one afternoon per week. The lead provider will have administrative and nursing support in the clinic, and oversee continuing care provided by a nurse or paramedic working in the field to follow up, monitor and assist patients with their care plan.

The ideal candidate should have strong clinical skills and be comfortable working in a low-resource setting, providing trauma-informed care for individuals facing significant medical and socioeconomic challenges.

Minimum: Certification as an Advanced Registered Nurse Practitioner in accordance with Chapter 464, Florida Statutes, and in accordance with Rule 64B9, Florida Administrative Code, or certification as a Physician's Assistant in accordance with Chapter 458, Florida Statutes.

Preferred: Experience working with high risk communities.
