Case Manager

Location: Wantagh | Schedule: Full-Time | Status: Non-Exempt | Salary: $24.73/Hour The Opportunity The Case Manager will provide crisis-oriented and preventive services to children and families experiencing difficulties at home to support improved family functioning and reduce family conflict. Responsibilities Conduct family and individual counseling sessions. Provide case planning and case management services for clients, connecting them with various resources as needed. Assess youth's appropriateness for the program and provide appropriate referrals if necessary. Implement preventive interventions to avoid neglect, abuse or a runaway situation at home. Work collaboratively with the Family Mediator Case Manager to ensure the appropriate service plan is developed and implemented. Assist youth in returning home when appropriate. Additional responsibilities as needed to ensure the effective operation of the program Qualifications Master's Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling or a related field required. A minimum of two years full time experience working with youth/young adults. Excellent verbal and written communication skills. Proficient computer skills with MS Office.
