Erie Family Health Centers in Chicago seeks a Birthing Families Navigator to join the Case Management Department. This role helps identify clients, delivers supportive services, and coordinates care from prenatal through early childhood with providers and care teams. You will perform outreach, assessments, referrals, home visits as needed, and maintain documentation in funder data systems and our electronic health record.
