Responsibilities Provide direct care for all assigned patients, initiating and maintaining communication with care team members. Educate patients/care partners to safely administer home infusion therapy. Assess the home environment for safety and identify necessary interventions. Collaborate with team members to develop and evaluate the plan of care. Maintain caseload scheduling via the company‑approved platform and communicate changes as needed. Coordinate care through the approved company platform. Participate in the nurse on‑call rotation to ensure continuous support. Document all communications at the time of service per Soleo policy. Demonstrate strong knowledge of therapies provided by Soleo. Work with departments to ensure all required patient documents are completed and returned on time. Ensure timely submission of plans of treatment as required by state guidelines. Schedule Part‑time: Monday‑Friday 8:30 am‑5 pm, 30‑32 hours per week, with variable schedule to include evenings/weekends as needed. Must be knowledgeable and experienced with IV access devices, accessing ports, and administration of infusion medications. Must be able to drive a properly registered/insured vehicle up to 60 miles one direction to see patients as needed. Must be able to participate in an on‑call rotation. Must be licensed as an RN in the state of California. Requirements RN degree; BSN preferred. Advanced certification in specialty areas (e.g., CRNI, ONS, IgRN) preferred but not required. On‑call requirement: participate in rotations, including nights and weekends. Administrative oversight responsibility: monitor workflows, ensure compliance, and manage resources. Emergency availability: respond promptly to emergencies, including nights, weekends, and holidays. Minimum 3 years nursing experience, with background in infusion therapy preferred. Current nursing license for applicable home state (California). Experience in ICU and/or cardiac care preferred. Proficiency with Microsoft Outlook and Office Suite. Valid state‑issued driver’s license with no significant restrictions.
