Highlights Pay: $72,627 - $155,538/year Location: Jamaica, NY Schedule: Full-time What You'll Do Support patient assessment and prioritization for walk-in and telephonic triage. Execute protocol-driven chronic condition management. Perform skilled nursing procedures including wound care and medication administration. Provide comprehensive patient and caregiver education regarding chronic conditions. Conduct medication reviews within nurse-led visits. Conduct onsite testing and use patient data to identify gaps in preventative screenings. Review and act upon high-priority abnormal laboratory results. Partner with providers to manage complex care plans and coordinate transitions. Utilize standardized protocols for medication management and prescription renewals. Delegate tasks to medical assistants and phlebotomists as needed.
