This position provides an opportunity to build meaningful relationships with patients and families while working as part of a collaborative hospice care team.The role is primarily field-based, with visits taking place in patient homes and healthcare facilities.Your RoleAs a member of the hospice team, you'll help patients and families identify their needs and connect them with appropriate support and resources.Your responsibilities will include:Evaluating the psychosocial needs of patients and their familiesDeveloping and providing appropriate social work interventionsOffering emotional support, counseling, and crisis assistanceHelping families navigate community resources and available servicesProviding guidance with advance care planning and related concernsSupporting patients and caregivers as they cope with changes associated with serious illnessAssisting families with anticipatory grief and bereavement needsAdvocating for patient preferences, dignity, and quality of lifeRecognizing and responding to cultural, emotional, social, and spiritual considerationsParticipating in interdisciplinary care planning with nurses, physicians, chaplains, and other cliniciansDocumenting assessments, visits, interventions, and follow-up in a timely mannerTraveling throughout the assigned service area to meet with patientsParticipating in the organization's on-call, weekend, and holiday rotationQualificationsRequiredBachelor's degree in Social WorkActive Ohio LSW or LISW licenseAt least one year of social work experience in a healthcare environmentValid driver's licenseReliable personal transportationAcceptable driving historyAutomobile insurance meeting state requirementsAbility to travel between patient homes and facilitiesComfortable using electronic systems for clinical documentationAdditional Experience We ValueMSW or other Master's degree in Social WorkPrevious hospice or palliative care experienceCHP-SW or ACHP-SW certificationCurrent CPR certificationNotary designation or willingness to obtain one within six months of startingBenefitsWe believe our clinicians should have the support they need both in the field and throughout their careers.
