Position Overview

This position involves coordinating and directing home nursing care as per physician's orders and agency policies. Responsibilities include applying the nursing process competently and supervising Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities

• Coordinates home nursing care in accordance with physician's orders and agency standards.
• Supervises and evaluates the performance of Home Health Aides and LPNs/LVNs.
• Conducts home visits and attends periodic office meetings.
• Ensures compliance with documentation requirements within 24 hours of patient visits.
• Maintains communication with the Clinical Manager regarding patient care and staffing needs.

Travel Requirements

The role requires reliable transportation, with a willingness to travel up to 75 miles from the home clinic for patient visits. Mileage will be reimbursed at the IRS standard rate.

Patient and Productivity Expectations

Contractor will be assigned 5-10 patients daily, leading to an expectation of 30 productivity points per week. Each type of visit and activity, such as documentation and travel, is assigned a specific point value.

Qualifications

• Minimum of 1 year of adult home health experience, or 1 year of hospice experience, depending on specialty.
• Current licensure as required.
• Certifications in CPR and BLS are mandatory.
• Proficiency in HomeCare HomeBase (HCHB) is required, with recent use within the last 6 months.
• OASIS proficiency is required for RNs, PTs, and OTs.

Work Schedule

The contract assignment duration is 13 weeks, with typical working hours from Monday to Friday, 8 AM to 5 PM. Weekend rotations will vary by location.
