Clinical Pharmacist – Home Infusion Location: Apex, NC 27502 Pay Rate: $64.27 per hour Schedule: Monday–Friday | Rotating Shifts (8:00 AM–5:00 PM, 8:30 AM–5:30 PM, 9:00 AM–6:00 PM) On-Call: Required evening and weekend coverage Overtime: As needed Type: Full-Time We are seeking a Clinical Pharmacist with strong home infusion experience to support a high-acuity patient care environment in Apex, NC. This role is focused on clinical oversight, sterile compounding, and end-to-end home infusion workflow management using CPR+. This position requires hands‑on clinical expertise, strong operational execution, and the ability to coordinate care across providers, nursing staff, and pharmacy technicians in a fast‑paced environment. Clinical Pharmacist – Home Infusion Role Overview The Clinical Pharmacist is responsible for managing home infusion therapies from referral through delivery. Responsibilities include clinical review, sterile compounding oversight, prescription processing, patient counseling, and coordination of care across multidisciplinary teams. This role also includes operational pharmacy support such as documentation, inventory management, claim processing, and technician supervision. Clinical Pharmacist – Home Infusion Key Responsibilities Clinical Oversight & Patient Care Review referrals for clinical appropriateness, including drug, dose, route, and lab requirements. Perform medication reconciliation for new and existing patients. Manage new patient starts and ongoing prescription refills. Provide patient counseling related to therapy, administration, and safety. Monitor lab-dependent therapies and elevate clinical concerns as needed. Coordinate care with physicians, nurses, and other healthcare providers. Pharmacy Operations & Workflow Accurately process prescriptions using CPR+. Support claim adjudication and ensure accurate documentation of patient records. Maintain compliance with pharmacy policies, procedures, and regulatory standards. Ensure timely processing of orders in a high-volume infusion environment. Sterile Compounding & Dispensing Prepare and verify sterile compounded medications in accordance with USP standards. Ensure accurate labeling, documentation, and lot tracking for all compounded products. Maintain strict adherence to sterility, safety, and quality control requirements. Team & Inventory Management Supervise and support pharmacy technicians in daily workflow execution. Maintain inventory of medications, supplies, and compounding materials. Support production scheduling and medication delivery coordination. Ensure efficient workflow across pharmacy operations. Clinical Pharmacist – Home Infusion Required Qualifications Active Pharmacist license in good standing Prior home infusion pharmacy experience (required) Experience with sterile compounding and USP compliance standards Familiarity with CPR+ or similar pharmacy systems Strong clinical judgment and medication management skills Ability to work in a fast‑paced, regulated healthcare environment Strong communication and coordination skills Preferred Qualifications Experience with IV infusion therapies and high‑acuity patient populations Background in hospital, specialty pharmacy, or home health settings Experience supervising pharmacy technicians Strong understanding of lab monitoring and therapy adjustments Experience with workflow optimization in pharmacy operations Ideal Candidate The ideal candidate is a clinically strong pharmacist with direct home infusion experience who can balance patient care responsibilities with operational execution.
