Job Details Job Location: Fort Bragg, CA 95437 Position Type: Full Time Education Level: High School/GED Salary Range: $21.00 - $28.40 Hourly Travel Percentage: Personal Vehicle Use Shift: Any Category: Nonprofit - Social Services Title: Care Manager Department: Crisis Response Services Status: Two (2) Full-Time, Non-Exempt Hours: 40 hours per week Open Date: April 1, 2026 Pay Differentials Language Translation - Bi-cultural Spanish: $1.00 per hour Bimodal Bilingualism - ASL Sign Language: $1.00 per hour Bimodal Multilingualism - ASL Sign Language: $2.00 per hour Overview Under general supervision, Care Managers assist in providing rehabilitative, restorative treatment services, crisis intervention, therapeutic intervention, assessment evaluation, and moderately complex, professional-level case management, including visiting with clients in residential or other settings, making referrals, and counseling clients on various issues, and providing one-on-one behavioral support. This position is based at one of our 24/7 residential programs and requires full schedule flexibility. Candidates must be available to work a variety of shifts, including morning (AM), evening (PM), and overnight hours. Key Responsibilities Demonstrate activities that address the purpose of the Rehabilitative group and individual client plan. Follow the State Mental Health and County Mental Health guidelines for any service being provided. Converse with clients to reinforce positive behaviors and to promote social interaction. Serve meals and eat with clients to act as a role model as it may apply to rehabilitative or behavior plans; assist with menu planning and grocery shopping with clients. Plan and execute regular scheduled activities or outings with the clients for purposes of skill-building. Provide medication support to clients as needed and complete required documentation. Qualifications Care Manager I: Less than 2 years of full-time experience in a mental health related field providing direct mental health services. Care Manager II: At least 2 years but less than 4 years of experience in a mental health related field providing direct mental health services. Care Manager III: At least 4 years of full-time or equivalent experience in a mental health related field providing direct mental health services; up to 2 years of experience can be substituted by years of education in a mental health related field. Minimum of 2 years actual work experience. Education requirement for Care Manager III: Minimum 2 years of education (60 semester or 90 quarter units) with at least 12 semesters (18 quarter) units in a mental health related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling. Conditions of Employment May work across all agency programs as needed. Occasional flexible work schedule required to meet individual client needs. Must possess a valid CA Driver’s License, current auto insurance, and a clean DMV printout (no more than 3 points). Must pass a clear background check with DOJ & FBI. CPR and First Aid Certified. TB Exam and Physical at the time of employment. Employees with direct responsibility for children under 18 or not emancipated from the foster care system, and employees of 24-hour residential care facilities for the elderly, blind or developmentally disabled individuals may be required to work on-duty meal periods without penalty when necessary to meet regulatory or approved program standards; one of the following two conditions must be met: The residential care employees eat with residents during residents' meals and the employer provides the same meal at no charge to the employee; or An employee, except for the night shift, may exercise the right to have an off-duty meal period upon 30 days' notice to the employer for each instance where an off-duty meal is desired, provided that there shall be no more than one off-duty meal period every two weeks. Driving Requirement Driving is a required function of this position. Employees must use a personal vehicle that meets all agency requirements, including maintaining liability insurance that meets or exceeds state requirements. Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements. Training Provided Comprehensive onboarding including CPR/First Aid, Crisis Communication/Pro-ACT, Trauma-Informed Care, Motivational Interviewing, and Collaborative Problem Solving.
