This position requires the position to travel between sites on a regular basis.Job Responsibilities:1.. Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors offices , health care providers, laboratories along with communicating and liaison with physicians , nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident. ( 65%)2. Track all physical, health care ,dental , vision appointment . (10%)3. Prepare and complete all necessary documentation in accordance with program standards within required time frames as required. ( ie. MA 51) (10%)4. Perform Direct Support Services when needed (10%).5. Work on computer to perform the above duties - ongoing6. Part of the after-hours on call system- OngoingThese are not all encompassed job responsibilities. Other duties may be assigned as necessary due to the needs of the program and residents served.Pre-employment screening:Complete criminal backgroundName checked in the registries.
