Duties include assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care. Responsibilities Evaluate home environment, assess for muscle strength, mobility, gait, and range of motion - potential for rehab. Instruct patients/clients and families/caregivers in the use and care of therapeutic appliances. Report patient/client reaction to treatment or changes in condition to the physician. Supervise homemaker/Home Health Aides when participating in patient's/client's rehabilitation regime. Prepare and submit clinical and progress summaries based on the attainment of goals. Comply with Medicare Conditions of Participation, OSHA, and other regulations as they pertain to position. Requirements Minimum of 2 years experience as a PT in an acute/home care/rehab setting. BLS Certification. Good oral and written communication skills; good organizational skills. Scrubs not required; dress code is slacks and polo or button-up shirt if scrubs are not worn.
