- Evaluate patients' functional abilities and develop individualized treatment plans.
- Administer therapeutic interventions to improve patients' daily living skills.
- Document patient progress and maintain accurate records in compliance with regulatory standards.
- Communicate effectively with patients, families, and healthcare professionals regarding care plans and progress.
- Ensure adherence to all safety and compliance guidelines during therapy sessions.
- Participate in on-call rotation and coordinate visits efficiently based on patient needs.
- Hold an active CA State Occupational Therapist license or pending application.
- Possess at least 1 year of relevant home health experience, with Oasis experience preferred.
- Demonstrate proficiency in documentation and clinical assessments.
- Maintain current BLS certification from AHA.
- Have a valid driver's license and personal transportation for patient visits.
- Ability to communicate effectively and work independently in a home health environment.
