Job Title: SSVF Shallow Subsidy Case Manager

Location: 1060 Howard St., San Francisco, CA 94103 (Primary)

330 Franklin St. Suite 100, Oakland, CA 94607 (As needed)

Schedule: Monday - Friday; 9:00 am - 5:00 pm or 8:00 am - 4:00 pm

Reporting Type: In-person, with possibility of one remote day per week after 90 days

Employment Type: Full-time; non-exempt

Salary Range: $30.77 per hour

Summary: The Shallow Subsidy Case Manager (ShaSub CM) is a key member of the Supportive Services for Veteran Families (SSVF) department, responsible for assessing and determining veteran eligibility for the shallow subsidy rental assistance program. Veterans currently served by traditional SSVF staff may be referred to this program. Once eligibility is confirmed, the ShaSub CM enrolls the veteran and begins providing case management services.

Serving clients from both the San Francisco and Oakland offices, the ShaSub CM delivers case management and rental subsidy support for up to 24 months. Using evidence-based practices, the ShaSub CM partners with veterans to achieve and sustain long-term housing stability. This includes working with community organizations, supportive housing programs, the VA VASH program, and market-rate landlords to expand housing opportunities and prevent evictions.

The ShaSub CM also collaborates closely with the broader SSVF team and the Drop-In Program to help veterans secure and maintain stable housing. This position reports directly to the SSVF Program Managers and/or SSVF Program Coordinators.

Responsibilities
  • Conduct client assessments - Complete intakes, eligibility screenings, and comprehensive assessments for veteran households referred to the Shallow Subsidy program.
  • Develop housing stability plans - Partner with veterans to create individualized plans that support long-term housing stability.
  • Provide ongoing case management - Maintain regular client contact, including monthly check-ins and quarterly financial reviews, to monitor progress toward financial and housing goals.
  • Coordinate supportive services - Make internal and external referrals to appropriate community resources and service providers.
  • Apply veteran-specific expertise - Maintain a strong understanding of veteran readjustment challenges, including PTSD, TBI, substance use, and other mental health concerns.
  • Ensure confidentiality and compliance - Follow all confidentiality and reporting requirements, including adherence to HIPAA policies and procedures.
  • Document services accurately - Complete timely and accurate progress notes, housing stability plans, aftercare plans, exit summaries, and all required SSVF documentation.
  • Manage program data - Enter and maintain client information in ECM Salesforce, including HMIS assessments.
  • Conduct housing inspections - Perform housing inspections as needed across San Francisco, Alameda, San Mateo, Contra Costa, and Marin Counties.
  • Provide financial coaching - Support veteran households with budgeting, money management, and financial planning.
  • Assist with benefits access - Help veterans establish disability income and access other benefits as needed.
  • Participate in team collaboration - Attend weekly team meetings, individual supervision with the Shallow Subsidy Program Manager, and other required meetings.
  • Perform additional duties - Carry out other related tasks as assigned by SSVF management.
  • Fulfill essential job functions - Maintain the ability to perform the essential duties of the position with reasonable accommodation.

Desired Qualifications

Experience
  • Direct service experience - Minimum of one (1) year working with "hard-to-serve" individuals (e.g., people experiencing homelessness, substance use disorders, or mental illness), particularly in housing stabilization, placement, or retention.
  • Data entry proficiency - Strong data entry and documentation skills.
  • Behavioral health knowledge - Basic understanding of mental health symptoms and diagnoses, psychosocial stressors, and chemical dependency issues.
  • Crisis intervention skills - Ability to de-escalate crises and resolve conflicts effectively.
  • Communication skills - Strong written and verbal communication abilities.
  • Relapse recognition - Ability to identify signs of relapse and respond appropriately.
  • Teamwork and independence - Ability to work independently while contributing effectively to a team environment.
  • Military experience equivalency - Relevant military experience and skills may be considered in place of traditional qualifications.
  • English communication - Ability to communicate effectively in English with staff, clients, and external partners.
Education
  • Degree preference - bachelor's degree in behavioral sciences preferred; relevant experience may be substituted.
  • Veteran services knowledge - Working knowledge or basic understanding of SSVF, HUD-VASH, and/or other veteran-focused services.
