Pay Rate: $1,500 – $1,900/week (Based on Experience)

Position Overview

We are seeking an experienced Licensed Master Social Worker (LMSW) with acute care/medical-surgical experience to provide psychosocial support, care coordination, and discharge planning for patients and families.

Required Qualifications

• Active LMSW (Licensed Master Social Worker) license eligible for practice in Washington.

• Minimum 2 years of professional social work experience.

• Mandatory acute care/medical-surgical experience.

• Master's degree in Social Work (MSW) from an accredited program.

• Strong knowledge of psychosocial assessment, discharge planning, and care coordination.

• Excellent verbal and written communication skills.

• Strong organizational, assessment, and critical-thinking abilities.

• Ability to work independently and effectively within a multidisciplinary healthcare team.

• Strong documentation and patient advocacy skills.

• Ability to manage a fast-paced hospital caseload.

• Willingness to provide occasional weekend coverage.

Responsibilities

• Conduct comprehensive psychosocial assessments and identify social, emotional, financial, and environmental needs.

• Provide social work services in an acute care/medical-surgical setting.

• Develop and coordinate appropriate discharge and transition-of-care plans.

• Identify and address barriers to safe discharge, including transportation, housing, financial concerns, caregiver support, and follow-up services.

• Provide emotional support, counseling, advocacy, and crisis intervention as appropriate.

• Connect patients and families with community resources and healthcare or social service programs.

• Collaborate with physicians, nurses, case managers, therapists, and other members of the multidisciplinary care team.

• Participate in patient care conferences and multidisciplinary team meetings.

• Communicate effectively with patients, families, caregivers, and healthcare professionals.

• Maintain accurate and timely documentation in the electronic medical record.

• Monitor patient needs throughout hospitalization and adjust social work interventions as appropriate.

• Advocate for patients while maintaining confidentiality and professional standards.

• Manage multiple patient cases while prioritizing urgent and complex needs.
