LTCR Nursing Facilities Investigator V

Date: May 15, 2026 Location: HOUSTON, TX Overview The Texas Health and Human Services Commission (HHSC) is seeking a dedicated Investigator V to conduct on‑site licensure inspections, complaint investigations, and annual surveys of long‑term care facilities across Texas. This front‑line field position involves frequent travel (up to 80%), the preparation of detailed deficiency reports, and occasional testimony at hearings. Responsibilities Conduct on‑site licensure inspections, complaint and incident investigations, and annual comprehensive surveys. Prepare inspection or survey reports that document deficiencies and recommend remedies. Communicate clearly with facility staff and stakeholders to exchange information, answer inquiries, and resolve issues. Conduct exit conferences with facility staff to discuss findings. Verify correction of previously cited regulatory violations during follow‑up visits. Perform quality assurance reviews of documents prepared by others. Testify at hearings or court proceedings as required. Assist with on‑call duties for emergencies such as weather events or high‑priority situations. Knowledge, Skills, and Abilities (KSAs) Knowledge of professional patient‑care standards and regulatory requirements. Ability to conduct investigations and verify compliance. Interpret policies, procedures, and regulations accurately. Ability to multi‑task and prioritize competing activities. Work independently with sound professional judgment. Proficiency in using computers to write reports. Operate personal computers and relevant software efficiently. Qualifications Bachelor’s degree from an accredited four‑year college or university. Experience in long‑term care or investigative work may substitute education on a year‑for‑year basis up to four years. Preferred experience writing detailed reports and operating personal computers and software. Preferred current license or registration as a nurse, social worker, respiratory therapist, or dietician. Must hold Surveyor Minimum Qualifications Test (SMQT) Certification or obtain it within 12 months. Background checks, including fingerprint criminal background check, required. Travel and Work Hours Requires up to 80% travel, including statewide, overnight, and weekend travel. Flexibility in work hours and on‑call duties for emergencies are mandatory.
