Physical Therapist

Role Overview: A licensed Physical Therapist providing personalized, in-home, senior living, and virtual care to older adults, focusing on improving mobility, independence, and quality of life. Key Responsibilities Conduct one-on-one evaluations and develop individualized treatment plans based on evidence-based practices. Deliver therapy sessions to support patient wellness, mobility, and functional independence in various settings. Document patient progress accurately within electronic medical records, adhering to regulatory standards. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Utilize telehealth and virtual care to enhance access and continuity of care. Qualifications & Skills Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited Physical Therapy program. Proficiency in basic computer and EMR systems. Current CPR certification. Physical Requirements Ability to perform moderate physical activities, including lifting, repositioning, assisting with transfers, and extended walking, as well as driving to patient locations and wearing PPE when necessary.
