Social Worker

W e are seeking a compassionate and dedicated Social Worker to join our care team. The Social Worker will provide emotional support, counseling, and resource coordination for patients and families, ensuring their psychosocial needs are met. This role is critical in helping individuals navigate complex health, social, and community challenges while promoting dignity, well-being, and quality of life. Key Responsibilities A ssess patients’ and families’ psychosocial needs and develop individualized care plans. Provide counseling, emotional support, and crisis intervention. Connect patients and families with community resources such as housing, financial assistance, and support groups. Collaborate with interdisciplinary care teams (nurses, physicians, therapists) to ensure holistic care. Advocate for patients’ rights and assist with navigating healthcare systems. Document assessments, interventions, and outcomes in compliance with organizational and regulatory standards. Facilitate support groups and educational sessions for patients and families. Required Qualifications Bachelor’s degree in Social Work (BSW) or related field. Minimum of 2–3 years of experience in healthcare, hospice, or community-based social work. Strong communication, counseling, and advocacy skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of local community resources and support services. Preferred Qualifications (if any) Master’s degree in Social Work (MSW). Prior experience in hospice, home health, or palliative care settings. Familiarity with electronic medical records (EMR) systems. Certifications (if any) State licensure or certification as required by practice location.
