This role is part of an interdisciplinary care team focused on delivering coordinated, patient-centered clinical care in facility environments.This position offers a strong work-life balance with no on-call, no weekends, and no holidays, along with flexible scheduling options.Primary ResponsibilitiesConduct comprehensive patient assessmentsManage complex acute and chronic medical and behavioral health conditionsEnsure accurate and complete documentation, including ICD-10 codingCollaborate closely with the interdisciplinary care teamLead advanced illness and advance care planning discussionsProvide patient and caregiver education and counselingParticipate in facility-based clinical activities and care planningRequired QualificationsActive and unrestricted state NP or PA license, or ability to obtain by start dateMaster's degree or higher (NP or PA)National board certification: NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP)PA: NCCPA certificationActive DEA licensure/prescriptive authorityAbility to lift a 30-lb clinical bag, climb stairs, kneel, and perform hands-on assessmentsReliable transportation for daily travelAbility to obtain a collaborative practice agreement if required in the stateFull COVID-19 vaccination (per federal, state, and facility requirements)Must pass pre-employment drug screeningPreferred Qualifications1+ year of clinical experience (long-term care preferred)Experience supporting patients with complex behavioral, social, or functional needsStrong understanding of geriatrics, chronic illness, and end-of-life discussionsProficiency in electronic and written medical documentationStrong communication, teamwork, and time-management skillsTravel DetailsTravel Required: Yes, during the work shiftFacility Count: 2Coverage Area: Wicomico & Accomack countiesInterview ProcessMicrosoft Teams interview
