Description:


The position involves providing home health and hospice care to a primarily geriatric population, with occasional pediatric cases.
Common diagnoses include heart disease, chronic respiratory diseases, CHF, COPD, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, surgery, and medication management.
Weekly interdisciplinary team meetings and case conferences are to be attended.
Monthly all staff meetings are also part of the role.
The RN will see 5-6 patients per day, totaling 25-30 points per week, performing routine and OASIS visits.
No vents are used; however, CPAP and BiPAP equipment are occasionally managed by patients themselves.
The RN will work Monday through Friday with potential variations in end times.
Weekend work is required during on-call shifts.
Call duty is scheduled for two times per week, Monday through Thursday.
There is no floating requirement, but the RN will perform both home health and hospice visits.
Solid color scrubs with closed-toe, non-slip shoes are required; patterns and logos are not permitted unless they are company-issued.
All clinicians are required to clock in and out at the facility.
The role may involve oversight from an administrator after hours, on weekends, and during holidays.
Candidates should have experience in hospice, particularly in pain and symptom management and providing compassionate end-of-life care.
