Clinical: RN Clinical Supervisor- Per Diem

All Metro Health Care , a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor- Home Care . You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.

Pay per Visit
- Sup/Post Hospital: $60
- Reassessment: $75
- Initial Admission: $90 weekday/ $125 weekend
Visiting Client Territory: Binghamton, and surrounding areas.
Schedule: Visits planned with the required presence of the patient's aide.
Office Address: 65 Pennsylvania Ave Suite 220, Binghamton, NY 13903
Mileage Reimbursement: Provided at the current federal rate

You will...

Performs on-site supervisory and teaching visits to evaluate caregivers
Provides clinical orientation to caregivers on assigned home care cases.
Participates in the training of new nurses and in-services for field staff.
Performs initial nursing assessments
Counsels patient/caregiver in meeting nursing and related needs.
Assigns and coordinates services provided.
Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.
Prepares clinical progress notes ensuring any changes in the patient's condition is reported
Monitors clinical records for complete and accurate documentation.

CPR Certified
1 year of Home Care experience required
1 year of supervisory or management experience required
Valid NYS Driver's License, insurance, registration, and reliable car
