We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director.A day in the life of a Conviva Primary Care Physician:Monday-Friday 8:00am-5:00pmJoin an experienced team with an established patient panelOpportunity to spend more time with patientsSpend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient careMinimal, shared call rotationEvaluates and treats center patients in accordance with standards of care.Follows level of medical care and quality for patients and monitors care using available data and chart reviews.Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.Acts as an active participant and key source of medical expertise with the care team through daily huddles.Helps Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.Completes all medical record documentation in a timely manner working with a quality-based coder to optimize coding specificity.Follows policy and protocol defined by Clinical Leadership.Use Your Skills to Make an ImpactDoctor of Medicine (MD) or Osteopathic (DO)Board certified or board eligible in Family Medicine or Internal Medicine - Geriatrics specialty would be idealValid, unrestricted license (or willing to obtain) in the state of desired employment (Florida)Current, unrestricted DEA licensePreferred Qualifications:Minimum of two to five years directly applicable experience preferredNew graduates will be consideredExperience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environmentKnowledge of Medicare guidelines and coverageKnowledge of HEDIS quality indicatorsBilingual (English and Spanish) is a plusAdditional Information:Time with your patientsCompetitive base salary + annual bonus incentiveMinimal CallComprehensive benefits package – health insurance is effective on your first day of employmentCME Allowance/TimeOccurrence Based Malpractice InsuranceGenerous Relocation Assistance availableSign-on bonus opportunity18 Days Paid Time Off (PTO) + 9 Paid Holidays + 1 VTO Day + up to 5 Personal Days (practitioners are offered 4 flex days per year)401(k) with Employer MatchLife Insurance/DisabilityCollaborative Team Environment and Career CultivationScheduled Weekly Hours: 40Pay Range: $255,000 - $298,500 per yearThis job is eligible for a bonus incentive plan.
