Job Title Behavioral Health Consultant II (Bilingual) Department Behavioral Health Reports to Director of Behavioral Health Benefits Medical, Dental, Vision, Life Insurance (Short & Long Term Disability) 403(b) Plan Paid Holidays CME (Continuing Medical Education) Location Work Location: Siler City CHC 224 South 10th Avenue Siler City, NC 27344 Schedule Monday 8:00am - 5:00pm, Tuesday 8:00am - 5:00pm, Wednesday 8:00am - 5:00pm, Thursday 8:00am - 8:00pm, Friday 8:00am - 1:00pm Travel: N/A About Position Behavioral health consultants (known as BHCs) work in collaboration with primary care providers and interdisciplinary primary care medical team to improve the overall health of the populations served at community health centers. BHCs provide screening, consultation, brief evidence-based interventions and treatment to manage emotional health and physical health behaviors as presented in the primary care clinic (e.g., sleep concerns, tobacco cessation, diabetes, depression, anxiety, substance use disorders). BHCs work via a stepped care approach to improve patient functioning using modalities appropriate to the primary care setting, and have expert knowledge on the specialty behavioral health services located within the community to promote health across the continuum. Duties / Responsibilities Co-manage health center patients with primary care providers and other clinical team members. Routinely conduct brief, evidenced based and developmentally sensitive screens for the alcohol and substance use. Routinely conduct brief, evidenced based and developmentally sensitive screens for depression and mental health problems. Expertise in common modalities used in primary care settings including: cognitive behavioral therapy, behavioral activation, motivational interviewing, solution focused, substance use and relapse prevention models. Recognize and diagnose, using established classification criteria within the DSM‑V of the most common mental health and substance use conditions. Recognize the signs, symptoms and treatments of the most common chronic health conditions, health crisis, and comorbidities Schedule includes individual appointments (20-30 minutes), group work as applicable, consultations with primary care team (exam room, curbside, team treatment planning). Document all work in shared electronic record, in a timely manner, that allows for unified care team communication and care plan. Collaborate with care coordinators about patient needs especially as related to community resources. Function independently and complete assignments with minimal supervision. Along with leadership, BHC will identify and manage specific population groups through clinical pathway programs (e.g., MAT, HIV+, homeless, pre and post‑partum, chronic care disease) BHC II will provide crisis assessment and intervention while in clinic, as applicable. BHC II will provide behavioral health expertise to primary care medical team. BHC II will engage in trainings each year to increase knowledge as needed for role including staying abreast of latest research. Qualifications Current/valid License: Fully NC‑licensed behavioral health clinician (e.g., Licensed clinical psychologist or licensed clinical social worker). Cross‑cultural training and Spanish language ability preferred. Strong problem‑solving ability and comfort with the inherent stress in fast‑paced, primary care clinics. Immunizations Be medically cleared for communicable diseases and have all immunizations up‑to‑date prior to beginning employment.
