SCHEDULE

• Most available shifts are 3 PM11 PM on weekdays and weekends.
• Per diem positions with flexible hours. Full-time or part-time hours may be available based on facility staffing needs.

POSITION DETAILS

• Position: Licensed Practical Nurse (LPN)
• Employment Type: Per Diem
• Location: Patchogue Suffolk County NY
• Setting: Facility Floor Staffing
• Pay Rate: $35$40/hour based on experience and assignment

RESPONSIBILITIES

• Provide quality nursing care and support to residents.
• Administer medications and treatments within the scope of the LPN license.
• Monitor residents conditions and report changes to the appropriate healthcare professionals.
• Maintain accurate and timely documentation using the facilitys charting system.
• Follow facility policies procedures and infection control protocols.
• Collaborate with other healthcare professionals to promote resident safety and well-being.
• Assist with other nursing duties as assigned within the scope of practice.

QUALIFICATIONS

• Valid New York State LPN license required.
• Previous nursing experience preferred.
• Long-term care or facility experience preferred.
• Strong communication and documentation skills.
• Authorization to work in the United States.
• Updated medical and vaccination records or willingness to obtain them.
• Completion of required drug screening and health documentation.
• Successful completion of Alvita Cares onboarding process before starting an assignment.
• Ability to meet facility scheduling and staffing requirements.
