Licensed Practical Nurse (LPN) Full-Time 8-Hour Night Shift

Summary Provides basic nursing care by assisting in the assessment of the resident at admission and on an ongoing basis. Remains knowledgeable about and communicates any significant change in condition to the appropriate interdisciplinary team member. Accurately documents all assessments and seeks guidance if unsure of any assessment findings. Participates in the care planning process, updates the care plan when interventions require revision, and communicates the plan to the appropriate IDT members, residents, and their families. Implements necessary and appropriate nursing interventions relevant to the plan of care, including medication administration and treatments, ensuring they are accomplished safely and timely. Administers IV fluid only if IV certified and documents interventions accurately and timely. Uses critical thinking skills when dealing with residents’ changes in condition, notifying the physician promptly using the SBARR communication tool. Maintains composure during crises, monitors residents closely to ensure effective delivery of the care plan, identifies problem areas, and uses the QAPI process for immediate problem resolution. Remains vigilant for behavioral changes or other signs and responds to call lights and assists with residents’ ADLs as needed. Completes at least twelve hours of nursing-related continuing education each year and stays current with best practices through knowledge of Bethesda’s policies and self‑study. May be assigned other duties as appropriate. Qualifications Education from an accredited school of nursing resulting in a Licensed Practical Nurse (LPN) license. Current LPN license in the state of employment. CPR and AED certification required, or willing to obtain within six months of employment. IV certification required, or willing to obtain within six months of employment. Long‑term care experience preferred. Ability to assess the basic nursing care needs of geriatric residents. Excellent communication skills to interact with residents, visitors, and staff. Time management and organizational skills required. Ability to stand and walk for long periods of time. Ability to lift heavy objects using proper lifting techniques and with assistance when necessary. Ability to transfer and move patients, including those weighing 200+ lbs, with proper lifting techniques and assistance when required. Ability to push and pull equipment weighing in excess of 40 lbs using proper techniques and assistance when necessary. Manual dexterity, coordination, and skillful use of hands when working with patients and equipment. Visual acuity with the ability to distinguish color.
