Start Date should be on or very quickly after the Order Start Date listed. All RTO should be listed on the cover page, must be less than 7 days. RTO will not be approved in the first 2 weeks of assignment. All facilities the traveler is open to working in should be listed on the submission. If interested in "any" location, please indlude what states (I.E. Any AZ, Any CO, Any state, etc.) All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. SUBMISSION REQUIREMENTS

• Minimum 24 months of current PCU experience - REQUIRED
• Certifications must be American Heart Association OR client and active upon submission - MUST BE INCLUDED IN SUBMISSION - REQUIRED
• No Nursys Hits - REQUIRED
• Flu vaccination - REQUIRED EFFECTIVE 9/16 **Please add vaccination status in the submission notes**

• Skills Checklist specific to the discipline (must be less than a year old)
• Active License (verification must be less than 30 days old)
• Certifications (must be active at time of submission)
• Flu vaccination eff 9/16 - must be willing to obtain
• Driver's License
