Job Description Job Description – Occupational Therapist (OT) Program CAPABLE (Community Aging in Place – Advancing Better Living for Elders) Report To Director of Nursing Position Overview The Occupational Therapist (OT) delivers home-based CAPABLE services focused on improving functional independence, safety, and quality of life for participants. Key Responsibilities Assessment & Goal Setting Conduct in-home functional and environmental assessments Use motivational interviewing to identify participant-driven goals Develop up to three (3) individualized Action Plans Intervention & Care Planning Address barriers to activities of daily living (ADLs) Recommend home modifications and adaptive equipment Develop and manage work orders for home improvements Care Coordination Collaborate with RN and interdisciplinary care team Align interventions with participant care plans Communicate progress, risks, and outcomes Participant Education Provide coaching to improve independence and safety Reinforce adaptive strategies and long-term self-management Documentation & Compliance Complete timely and accurate documentation Maintain compliance with CAPABLE protocols and CABS standards Qualifications Licensed Occupational Therapist (New York State) Minimum 1 year of experience (home care preferred) Strong patient engagement and communication skills Standards of Performance (OT) Demonstrates client-centered, goal-driven care Conducts thorough and accurate assessments Maintains strong interdisciplinary communication Ensures all recommendations are safe and appropriate Completes documentation in a compliant and timely manner Demonstrates accountability in managing caseload Key Performance Indicators (KPIs – OT) ≥90% visit completion within program timelines 100% documentation completed within 48 hours ≥80% improvement in participant ADLs ≥90% reduction in environmental risks identified ≥85% participant satisfaction ≥90% participation in case coordination activities Company Description CABS Health Network, (CABS) is a well-established Licensed Home Care Agency located in Brooklyn, New York, an innovative home healthcare agency with a 40-year history in the home healthcare industry.
