Kidspath, a service of AuthoraCare Collective, is currently seeking an RN Home Care Case Manager focused on home health in our Greensboro service area .


This position is full-time , hours flexible. Willing to train to the skillset for someone with experience in pediatrics. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Jo b Summary:

Manage RN case load for all/multiple services of Kids Path program (may include hospice, home health, perinatal services), in collaboration with other members of the interdisciplinary Team (IDT). Assess and identify patient/caregiver complications, establish patient/caregiver care goals and identify and implement interventions. Work with physician to establish and implement medical plan of care for the patient.


Establish and maintain therapeutic relationship with patient and caregiver. Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Availability after-hours occasionally required at the discretion of the Kids Path leadership team. Program support responsibilities required in combination with field responsibilities. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Requirements

Required Qualifications:

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• At least one year of experience required (two years preferred) in an acute or home setting.
• At least one year of pediatric experience required.
• Bachelor's degree in nursing preferred.
• Familiarity with Medicaid, and/or home health a plus.
