A growing Skilled Nursing Facility is seeking an energetic, compassionate, and driven Physical Therapist (PT) to join its rehabilitation team. 

This role is perfect for a Physical Therapist who is passionate about helping residents improve mobility, regain independence, and enhance their quality of life in a supportive long-term care environment.

Position Responsibilities:

• Perform physical therapy evaluations and create customized treatment plans
• Deliver high-quality therapy services to short-term rehab and long-term care residents
• Focus on improving strength, mobility, balance, and daily function
• Collaborate closely with nursing, therapy, and interdisciplinary teams
• Educate residents and families on treatment plans and progress
• Complete timely and accurate documentation

Qualifications:

• Active Physical Therapist license in KY
• Skilled Nursing or geriatric therapy experience preferred
• Strong clinical and communication skills
• Positive, team-oriented attitude
• Passion for resident-centered care
