Hospice Social Worker (MSW/LCSW) – Full-Time Contract 13-Week Contract with Potential for Extension or Permanent Placement Community Health Network (CHN) has specialized in placing experienced healthcare professionals in hospice, home care, and case management settings for over 30 years. We are currently seeking a compassionate and experienced Hospice Social Worker (MSW/LCSW) to join a well-established and supportive hospice team serving patients throughout the Merrimack, Nashua, and Hampton, NH areas. This is an excellent opportunity for a dedicated social worker who is passionate about providing emotional support, advocacy, and guidance to patients and families navigating end-of-life care. As a member of the interdisciplinary hospice team, the Social Worker reports directly to the Clinical Manager and is responsible for assessing psychosocial needs, coordinating supportive services, and providing counseling and emotional support to terminally ill patients and their families. The ideal candidate is compassionate, organized, emotionally resilient, and comfortable working with issues related to grief, death, dying, and end-of-life care. Conduct psychosocial assessments for hospice patients and families Develop individualized care plans in collaboration with the interdisciplinary team Provide emotional support, counseling, crisis intervention, and bereavement resources Assist patients and families with coping strategies related to terminal illness and end-of-life transitions Coordinate community resources, financial assistance programs, and supportive services Advocate for patient rights, dignity, and quality of life Educate patients and caregivers regarding hospice philosophy and available support services Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards Participate in interdisciplinary team meetings and care conferences Travel throughout assigned coverage territory for patient visits Qualifications Master’s Degree in Social Work (MSW) required Active New Hampshire Social Work license required (LICSW preferred) Minimum 1–2 years of Hospice, Palliative Care, Home Health, or related medical social work experience preferred Strong communication, assessment, and crisis intervention skills Comfortable working with terminally ill patients and discussing end-of-life care Ability to work independently while collaborating effectively within a team environment Reliable transportation and valid driver’s license required Schedule Full-Time Monday–Friday 40 hours per week Competitive weekly pay Flexible scheduling and strong work-life balance Health Insurance Vision Insurance Life Insurance 401(k) Retirement Plan Opportunity for contract extension or permanent placement #J-18808-Ljbffr Frey Consulting Group
