PRN Social Worker

The incumbent offers short‑term therapeutic counseling and crisis intervention to patients and families with psycho‑social needs, and coordinates and facilitates the development of a discharge plan of care for high‑risk patient populations, including accountability for the complex discharge process. This position ensures an exceptional experience of care for the patient and family. Responsibilities Accountable for multiple performance and utilization risk components such as: patient flows throughout quality initiatives coordination of care care transitions Position duties include: Conduct psychosocial assessments within 72hours of admission, documenting history and identifying preliminary issues for treatment focus by interviewing the patient and family members. Interpret, support, and execute hospital and department policies and procedures relative to care provision in the social services setting. Communicate with physicians regarding patient progress, needs, concerns, and any potential or actual problems. Collaborate on the development of the Master Treatment Plan based on assessments and accepted social work interventions, completing it within 72hours of admission and updating it within 7days. Provide group and family process‑oriented therapies, conduct educational and didactic groups, and promptly complete ongoing progress notes. Deliver short‑term individual counseling, community resource planning, and crisis intervention. Create individualized discharge plans by consolidating treatment team interactions and chart information to determine after‑care needs. Attend regular treatment team meetings to provide a social work perspective on case management, including reviewing progress notes and communicating legal requirements. Maintain knowledge of referral resources throughout the service area and act as a resource person for the team, providing in‑service education and consultation. Stay current on Medicare, Medicaid, and other third‑party reimbursement sources. Maintain accurate and timely clinical and progress notes, completing all necessary filing by the close of each business day. Participate in continuing education to keep knowledge up to date. Provide positive customer relations with patients, families, physicians, staff, other departments, referral resources, and service agencies. Update patient information at least twice per week in person or by telephone to Guardian/POA or other identified family contact, and to each patient’s referring facility. Provide backup for incoming assessment and referral calls. Float to other departments or facilities as directed. Maintain confidentiality and privacy of health information, ensuring protection through written, electronic, or oral disclosures. Be prompt and efficient with minimal absences. Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors. Seek guidance and comply with all applicable federal and state laws and hospital policies. Adhere to hospital expectations regarding ethical behavior and standards of conduct. Identify research, best practice, and data‑driven opportunities for improvement in new and existing work processes. Partner with physicians in all aspects of patient care and coordination, engaging patients and families in the plan of care. Communicate and collaborate with all team members to ensure safe and timely delivery of patient services, providing necessary resources and education. Collaborate with the healthcare team to mitigate risk to patients, families, visitors, employees, physicians, and the system. Perform other duties as assigned. Minimum Qualifications Education : Master’s Degree in Social Work or a related field. Experience : 3–5 years of experience as a social worker, with preference given to mental health experience. Preferred : Licensed Independent Social Worker or Licensed Clinical Social Worker.
