Job Description
Title: Home Health Registered Nurse - RN

Schedule:
- Shift: 8-hour days
- Weekends: Not required

Assignment Details:
- Patient ratio: 1:1, with potential to visit up to 6 patients per day

Requirements:
- Minimum 1 year of skilled home health RN experience
- Required certifications: BCLS
- Required licensure: RN licensure in the state
- OASIS experience preferred

Roles & Responsibilities:
- Provide skilled nursing assessments and care in home health settings.
- Administer medications and perform treatments as prescribed.
- Educate patients and families on care plans and health management.
- Manage wound care, including wound vac and PICC line care.
- Collaborate with interdisciplinary teams to optimize patient outcomes.

Compliance / Notes:
- Orientation: 40 hours
- Confirm any restrictions on time-off with the hiring manager
- General attire: Business casual preferred; scrubs allowed but not required, no jeans
