Responsibilities
- Perform comprehensive PT evaluations and develop individualized plans of care for residents
- Implement therapeutic interventions including gait training, therapeutic exercise, balance training, ROM and strength work
- Apply modalities as appropriate, monitor patient progress, and modify plans as needed
- Document sessions accurately, maintain compliant charting, and communicate with the interdisciplinary care team
- Participate in discharge planning and contribute to safe, evidence-based care within a long-term care setting

Requirements
- Active physical therapy license in good standing; licensure as required by the state of practice
- Doctor of Physical Therapy (DPT) degree or equivalent
- Experience in skilled nursing facility or geriatric rehabilitation settings preferred
- Ability to work in a long-term care environment and collaborate with an interdisciplinary team
- Strong documentation and communication skills; capacity to commit to a 15-week travel assignment
