Location Manhattan Schedule Full time Key/Essential Functions & Responsibilities Responsible for ensuring safe and appropriate nursing services and medication administration for persons supported across assigned caseload, which may include a variety of program(s) and/or system(s), working in collaboration with regional, nursing and other leadership and interdisciplinary teams to ensure implementation of person-centered, individualized plans of nursing service by regularly monitoring, assessing and addressing program and person-specific needs through oversight of nursing staff. Serves as the primary resource for day-to-day nursing and medical-related needs of the people we support across assigned caseload, providing leadership and clinical support to program staff, regularly attending regional leadership meetings to remain informed of progress, concerns or issues, making recommendations and implementing resolutions to address new or changing needs. Develops and maintains an understanding of regulations applicable to assigned programs and/or systems as they relate to medical and nursing care, including needs or care for specific medical and related conditions of persons supported and of scopes and standards of nursing practice applicable to state(s) of assignment. Directly supervises Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for assigned caseload across or within a region, providing support and guidance for nursing services to ensure that the medical needs of persons supported are successfully met. Maintains regular communication with assigned nursing staff to ensure understanding and consistent application and enforcement of job expectations, performance standards, policies and procedures and conducts regular observations, including on-site at programs or systems across assigned caseload, to evaluate performance, managing concerns of performance issues through discussions and plans for corrective actions or development plans, as needed. Ensures nursing staff are utilizing the full nursing process with each person supported from assessing conditions through improving effectiveness of care and are appropriately communicating with persons supported based on their learning needs, as they relate to health care, to support understanding and education relating to individualized medical treatments, medications and related information. Regularly monitors and assesses nursing caseloads based on program and/or person-specific needs, proposing changes and new needs to nursing leadership team, including reassignments of Regional HCS to programs across caseload to address specific needs or concerns. Directs and assists assigned nursing staff with implementing, monitoring and otherwise updating plans of nursing care or PONS, addressing new and changing medical needs for persons supported and handling complex medical issues through regular support, feedback and coaching, on site, in person and virtually as appropriate. Regularly travels to programs across assigned region to engage with or observe persons supported, program staff and nursing staff or to conduct assessments, implement plans, collect data, facilitate medical in-services and/or attend meetings to ensure appropriate implementation and monitoring of nursing plans; travels to local headquarters or other regions, as directed. Participates in the hiring process, interviewing and assessing candidates for nursing roles and ensures appropriate staffing and coverage for assigned caseload and assists with the identification of partnerships and with or at recruitment initiatives or events to attract and retain nursing staff, in collaboration with HR and nursing leadership teams. Initiates, reviews or approves staff status, work hours, promotions, demotions, transfers and/or other adjustments for staff, ensuring changes are appropriately submitted in Workday in a timely manner. Ensures timekeeping records and approvals are completed through Workday accurately and on a timely basis to support payroll processing; identifies concerns with attendance, work hours and/or time tracking, resolving with staff or coordinating resolutions with leadership, when needed. Collaborates with regional program leadership, program staff and nursing team to identify, address and/or coordinate clinical training needs, supports, assessments, documentation and related plans, providing feedback regarding program staff and people supported, as applicable. Plans and facilitates regional direct care leadership and/or other clinical team meetings to engage, train and assess Health Care Specialist and other direct care staff as applicable, providing support, guidance, mentorship and feedback through regular updates, information and discussions; directly supervises Regional Health Care Specialist(s), assigning responsibilities and monitoring performance. Regularly monitors medical systems, including Electronic Medication Administration Record (eMAR), to ensure quality of documentation and adherence to prescribed orders and/or requirements for services including medication administration and confirming appropriate, complete, person-centered and timely documentation of all plans, calls, correspondence and encounters with people we support in appropriate formats and systems, in accordance with all established guidelines and regulations. Supports nursing leadership with the implementation and communication of new and updated codes, rules, regulations, administrative memoranda and/or protocols for nursing/clinical practice, including localized infection control practices and other related plans for communicable diseases, OSHA regulations and exposure control by providing guidance and education to assigned caseload and escalating concerns or issues. Reviews medical data for assigned caseload through regular monitoring of electronic health record, eMAR and/or other systems to support quality assurance of nursing-related services, conducting regular checks of and analyzing documentation and information to identify and report on key indicators, such as medication errors, 911 calls and falls, summarizing trends and changes over time. Collaborates with applicable parties to address audit findings and other issues or concerns, actively participating in the development and implementation of plans of corrective action to resolve findings with a focus on continuous improvement, directing activities, facilitating specialized or ad hoc trainings to address deficiencies, conducting regular check-ins with program to assess effectiveness and making recommendations for changes to improve health outcomes for persons supported. Participates in nursing call list for assigned caseload, receiving and triaging phone calls as needed, including escalated calls from nursing staff and assists with handling or directing calls from program staff, leadership and/or persons supported during and outside of regular working hours. Presents information and responds to inquiries regarding health, conditions and/or status of persons supported across assigned caseload and assists with nursing-related needs in other service settings or systems as needed or requested, which may include participating in meetings, planning, assessments and/or education. Ensures appropriate oversight of medical care and systems for assigned caseloads by nursing staff, including regular in-service training and education for direct care staff across assigned caseload regarding medical needs, medication information and administration or other conditions including short-term needs for specific conditions through regular monitoring. Takes the lead for intake assessments during the referral process for assigned caseload, reviewing medical records and documentation, suggesting matches to programs and ensuring or facilitating direct observations and/or assessments to inform recommendations to program and/or nursing leadership for placement. Participates in residential nursing on-call system on a regularly scheduled, rotational basis or as applicable for assignment to ensure appropriate nursing coverage and response to incidents, inquiries or emergencies, including on a 24/7 basis within 30 minutes in accordance with OPWDD guidelines and/or as prescribed for other program, system or agency requirements. Keeps abreast of applicable Federal, State and/or Local government regulations and standards, including those applicable to oversight agencies for assigned setting(s), infection control and/or nursing care and completes and maintains all necessary trainings based on assigned system(s) to ensure compliance with all applicable regulations and internal policies or procedures. Provides support to the agency as a whole, participating in various program and nursing-focused initiatives and committees requiring nursing involvement such as Human Rights, Incident Review and Choking Committees and/or supporting additional programs, systems and special projects, as requested. Participates in the development of medical in-service trainings or assessments for specific content areas such as AMAP, choking and diabetes and/or for specialized programs including START, OMH, etc. in collaboration with nursing leadership team. Complies with and ensures staff complies with all Federal, State, Local and other relevant regulatory agency requirements, including the Health Insurance Portability and Accountability Act (HIPAA). Performs all other duties as assigned. Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.) Valid, active Registered Nursing (RN) license and/or registration to practice as an RN in assigned state of operations (NY or NJ); and Holds and maintains Basic Life Support (BLS) certification for Healthcare Providers; and Bachelor of Science in Nursing (BSN) degree and three (3) years of nursing experience in any healthcare setting, preferably working with individuals with intellectual and developmental disabilities (I/DD); or Satisfactory equivalent combination of education, experience and/or training; however, all incumbents must hold current, valid license to practice as a RN in state of assignment and current BLS. Demonstrates sound clinical judgment in selecting methods and techniques to resolve medical concerns. Excellent interpersonal, verbal and written communication skills, along with the ability to effectively interact with and communicate medical information to persons supported, staff at all levels and circles of support, including external medical providers. Exceptional supervisory and leadership skills, including the ability to effectively direct, coach and support nursing and program staff across multiple sites through hands‑on support by adapting leadership style to address a variety of team and/or individual challenges. Ability and willingness to lead, direct and teach program staff on appropriate medical support for persons supported, including medication administration and specialty care for complex or specialized needs, both on‑job and through formal training in group or individual settings. Proficiency with computers and experience using electronic medical record, billing and/or other related electronic systems, email and Microsoft Office Suite, along with the ability to quickly learn new systems and programs, including Workday. Highly organized and flexible with ability to effectively manage multiple priorities to ensure adherence to all applicable requirement and deadlines, while appropriately addressing a variety of person‑centered needs. Must obtain and maintain any additional certifications as required for assigned programs, systems and/or persons supported, which may include Strategies for Crisis Intervention and Prevention – Revised (SCIP‑R), Diabetes Care or Train‑the‑Trainer trainings. Ability to regularly report to programs in assigned caseload and to travel to, from and/or between programs, as needed and to other regions or local headquarters, as requested. Ability to work a flexible schedule as required by caseload and to respond to and/or triage calls regarding medical concerns or emergencies for assigned caseload outside of regularly scheduled working hours and to be on‑call, as needed. Ability to meet essential physical demands of position including frequent walking, sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling and reaching with hands and arms; use hands to handle, finger or feel objects, tools or controls; assisting with lifting/moving individuals of any weight with assistance of mechanical lifts or other equipment and/or pushing individuals in wheelchairs; lifting and/or moving up to 25 pounds on occasion; having the physical capacity to work with and implement emergency interventions as per the person’s Behavior Support Plan if/as needed, including lifting/moving individuals of any weight with or without assistance of equipment and/or other staff. Preferred Qualification Requirements One (1) year of experience supervising nurses in a formal or head nurse capacity. Two (2) years of experience supporting people with I/DD in any capacity including in a nursing home, assisted living facility, rehabilitation center and/or setting applicable to assigned caseload. Knowledge of I/DD, related diagnoses and applicable regulations in state of assignment (OPWDD, DDD, etc.).
