Job Title: Certified Occupational Therapy Assistant (COTA)

Location: Florence, WI | Skilled Nursing Facility

Schedule / Job Type: Contract | Days | 13-Week Assignment

Key Responsibilities:

• Provide occupational therapy treatment services to residents under the supervision of a licensed Occupational Therapist
• Implement individualized treatment plans in a skilled nursing facility setting
• Document patient progress and maintain accurate therapy records in accordance with facility and regulatory standards
• Collaborate with the interdisciplinary care team to support resident goals and outcomes

Required Qualifications:

• Current COTA licensure or certification required
• Active state licensure or eligibility to practice in Wisconsin
• Experience working in a skilled nursing or long-term care environment

Preferred Experience / Skills:

• Prior SNF or long-term care experience preferred
• Familiarity with Medicare documentation and compliance requirements
