Community Wellness Specialist - ACT Team

sus.com seeks a Wellness Specialist in New York to provide vital services as part of a mobile multidisciplinary treatment team. This role includes participating in daily meetings, providing support, and developing wellness interventions tailored to participants' needs. The ideal candidate holds a Master's degree in social work or psychology and has excellent communication skills. Responsibilities also include documentation and facilitation of service plans.
