A-Line Staffing is now hiring Hybrid Behavioral Case Manager located in Peoria County, IL . This would be full time / 40+ hours per week.
Hours: Monday – Friday, 8am-5pm, no weekends, no holidays
Pay: $34.13 per hour

Hybrid Behavioral Case Manager Highlights

·         Hybrid role with 50-75% travel to meet with members face-to-face in Illinois.

·         Willingness and ability to travel extensively, with reliable transportation eligible for mileage reimbursement.

·         Field-based position managing a full caseload of waiver members with quarterly in-person visits.

·         Compliance with contractual requirements and regulatory guidelines.

Hybrid Behavioral Case Manager Responsibilities

·         Conduct comprehensive evaluations of members' needs and eligibility using care management tools and data review.

·         Identify high risk factors and service needs, recommending case resolution strategies and referrals as appropriate.

·         Coordinate and implement care plan activities, monitoring progress and adjusting as necessary.

·         Consult with case managers, medical directors, and health programs to overcome barriers and achieve optimal outcomes.

·         Identify and escalate quality of care issues through established channels.

·         Utilize negotiation and motivational interviewing skills to engage members and promote health and lifestyle changes.

Hybrid Behavioral Case Manager Requirements:

• Valid Illinois Driver’s license.
• Minimum of two (2) years of case management experience.
• High school diploma or GED required; bachelor’s degree or master’s degree in a human-services field preferred.
