Inpatient Social Worker

Kaiser Permanente seeks a Licensed Social Worker for Inpatient Social Services within our Healthcare & Medical Services department. In this on-site role, you’ll provide psychosocial assessments, crisis intervention, discharge planning, and care coordination for hospitalized patients and families. Collaborating closely with an interdisciplinary team, you will advocate for patients, link them to community resources, and support safe transitions of care.
