Care Manager MSW at Providence St. Joseph Medical Center in Burbank, CAThe Inpatient Master of Social Work (MSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy.
